Here comes the full support for AVerMedia Cardbus Plus (E501R) - including
remote control. TV, Composite and FM radio tested, I don't have S-Video to
test. I've figured out that the radio works only with xtal frequency 13MHz.

to new_(probed)_subdev
The functions v4l2_i2c_new_subdev and v4l2_i2c_new_probed_subdev relied on
i2c_get_adapdata to return the v4l2_device. However, this is not always
possible on embedded platforms. So modify the API to pass the v4l2_device
pointer explicitly.

Added support for I2C device at address 0x40 and subaddress 0x0d/0x0b
that provides remote control key reading support for AVerMedia Cardbus
Hybrid card, possibly for other AVerMedia Cardbus cards.
The I2C address 0x40 doesn't like the SAA7134's 0xfd quirk, so it was
disabled.

i2c gate
Some saa7134 devices require to open an i2c gate before tuning. This
patch fix the initialization for those devices.
The nxt200x_gate_ctrl() logic were returned back to the old place, since
we don't know how to close the gate. A future pacth could revert that
change and provide the proper close gate control, to avoid keeping it
open forever.

On certain devices, before opening a tuner, we need to open the tuner
gate via i2c.

such i2c commands are handled, to make easier to fix this trouble on

The IR controller has a couple quirks. It won't respond until some other
device on the bus is probed. To work around that, probe 0x50 first.
Then, since it won't respond to a zero-byte read, probe with a one-byte
read.

overwriting manual tuner type
When saa7134_board_init2 runs, it immediately overwrites the current value
(set earlier from module parameter) of tuner_type with the static values,
and then does autodetection. This patch moves the tuner_addr copy to earlier
in saa7134_initdev and removes the tuner_type copy from saa7134_board_init2.
Autodetection could still potentially change to the wrong tuner type, but it
is now possible to override the default type for the card again.
My card's tuner is configured with autodetection from eeprom, so I don't
need to manually set the tuner. I've checked that the autodetection still
works for my card.

Tuner setup were happening during i2c attach callback. This means that it would
happen on two conditions:
1) if tuner module weren't load, it will happen at request_module("tuner");
2) if tuner is not compiled as a module, or it is already loaded
(for example, on setups with more than one tuner), it will happen
when saa7134 registers I2C bus.
Due to that, if tuner were loaded, tuner setup will happen _before_ reading
the proper values at tuner eeprom. Since set_addr refuses to change for a tuner
that were previously defined (except if the tuner_addr is set), this were
making eeprom tuner detection useless.
This patch removes tuner type setup from saa7134-i2c, moving it to the proper
place, after taking eeprom into account.
